The district attorney&#8217;s office tells KTBB, Chasity Khan, 32, of Tyler, was indicted on May 12 on one count of intoxication manslaughter. The accident happened just before 10:00pm on December 1st on County Road 2120. Her vehicle ran off the road and hit several trees. Khan was hospitalized. A passenger in her vehicle, Jason David Cook, 37, was pronounced dead at the scene.
