According to our news partner KETK, on June 5, members of the Tyler Police Department were dispatched to a home in the 3100 block of Cascades Court after receiving reports of a suspicious person on a property.

According to the report, a homeowner stated that she was currently getting divorced from a man identified as John O’Kelly, and that he was given a court order restricting him from returning to the home.

The homeowner was out of the house at the time but told officers that O’Kelly had returned to the home and was seen on home security cameras assaulting her father. The report stated that one officer arrived on the scene, observed damage to the backdoor and began attempting to negotiate with O’Kelly to go outside the home.

It was also revealed during negotiations that O’Kelly had a history of using methamphetamine and alcohol, and that he had been kicked out of a rehabilitation facility earlier in the day, according to the report.

As officers attempted to peacefully remove O’Kelly from the house, O’Kelly stated he had a gun and threatened to shoot officers, the report described. The homeowner was able to watch what O’Kelly was doing inside the home off of a live video feed from a security camera system and relayed what he was doing to the authorities.

After the homeowner informed officers that O’Kelly had created a noose and placed it around his head, the report stated that officers entered the home and ordered O’Kelly to show his hands as they began to go upstairs to arrest him.

“While officers ascended up the stairway, O’Kelly emerged at the top of the stairway, and presented an object, later determined to be a black electrical razor. O’Kelly held the black razor in a two hand-grip and thrust forward the remote, presenting it as a firearm. The lead officer climbing the stairway, believed the item to be a firearm and shot 4 rounds from his patrol rifle, striking O’Kelly at least two times in the chest area.”

After the noose was removed from O’Kelly’s neck, officials began life saving measures and he was transported to a hospital in Tyler where died later that day from his injuries. The Texas Rangers were notified and have began an investigation into the shooting. Additional results are pending after autopsy that was ordered on O’Kelly’s body.

TYLER — A custodial death report from the Texas Attorney General’s office has released more information about a fatal officer-involved shooting that occurred in the Cascades earlier this month.
